Position Overview
We are seeking an experienced RN Nurse Case Manager to join a Federally Qualified Health Center (FQHC) network on the south side of Chicago. This hybrid position is a great chance to make an impact while working with a dynamic, interdisciplinary team. In this role, you will coordinate patient care, ensure compliance with clinical policies, and help manage high-risk patients, including those with obstetrical and perinatal needs. This position offers a 60% remote and 40% on-site work schedule, with hours from 8 AM to 5 PM, Monday through Friday. Additional overtime is paid at time and a half for hours worked beyond 40 hours per week.
Key Responsibilities
Develop individualized care plans for patients, performing risk assessments and assigning appropriate levels of care to all patients in your caseload.
Collaborate with a diverse care team, including Medical Assistants, Providers, Behavioral Health, Dental, Nutrition, WIC, Healthy Start, and others, to manage and coordinate patient care.
Promote and facilitate patient referrals to additional support services such as dental care, nutrition programs, and behavioral health.
Track patient appointments, monitor for no-shows, and re-establish care for continuity.
Oversee the management of multiple chronic conditions for patients, ensuring follow-up care and appropriate treatment plans.
Communicate treatment plans and provide patient education, ensuring that patients understand their care process and options.
Liaise with Labor and Delivery departments at affiliated hospitals to ensure continuity of care and discharge planning.
Prepare patients for delivery by coordinating hospital tours and pre-birth visits.
Provide post-delivery services including newborn support, family planning, and education on inter-conceptual spacing.
Participate in case discussions during weekly rounds with the Chief Medical Officer and other team members, as well as the monthly Women's Health meetings.
Maintain frequent contact with patients during all trimesters and through 12 weeks post-delivery to ensure adherence to care plans.
Document all patient interactions and services in the Electronic Health Record (EHR), ensuring complete and accurate records from initial care through 12 weeks postpartum.
Follow up on high-risk patients, ensuring that transfers to tertiary centers and post-delivery care are managed effectively.
Qualifications
Bachelor's degree in Nursing required.
Valid RN license in the state of Illinois.
Minimum of two (2) to five (5) years of acute nursing experience preferred.
Experience in case management, particularly with obstetric patients, is highly desirable.
Strong communication, interpersonal, and organizational skills.
Flexible, energetic, and customer-service oriented.
Current CPR Certification required.
Bilingual skills are a plus.
Proficiency in computer systems for documentation and care management.
