Background

Since 1976, St. Joseph Center has been meeting the needs of low-income and homeless individuals and families in Venice, Santa Monica, Mar Vista, and surrounding communities. The Center is a 501(c)(3) nonprofit community organization that assists people without regard for religious affiliation or lack thereof through comprehensive case management and integrated social service programs. The Center enjoys broad-based community support as well as a sponsored relationship with its founders, the Sisters of St. Joseph of Carondelet. St. Joseph Center serves approximately 6,000 individuals annually.

Brief Description of Principal Activities

Multifaceted intervention, prevention, and education services are carried out at four sites on the Westside of Los Angeles. St. Joseph Center’s integrated programs provide clients with concentrated and coordinated access to services according to the nature of their needs.

Job Summary

The Case Manager serves as a team member with several social service professional and paraprofessional staff ensuring the delivery of quality as well as adhering to the mission of St. Joseph Center. The Case Manager will provide case management to homeless families experiencing domestic violence including service linkage and housing support.

ESSENTIAL DUTIES & RESPONSIBILITIES

The Case Manager’s key areas of responsibility include:

  • In conjunction with the Program Manager and LAHSA’s DV Unit case management staff, evaluate families for permanent housing.
  • Perform advocacy on behalf of the participant; Conducts intakes and exits with families entering and exiting the program.
  • Case manager will practice Trauma-Informed Care, Housing First, Harm Reduction, and Low Barrier techniques when working with participants.
  • Be responsible for a caseload of up to 20 families. Establish a case management and safety plan for each family; develop housing retention plans to address short-term and long-term housing goals.
  • Work closely with the DV Regional Coordinator and the LAHSA DV Unit.
  • Assists with identifying internal and external resources to assist with multiple stressors as they relate to homelessness, domestic violence, substance abuse issues, etc.
  • Participates in case conferences, case consultations, and other client-related meetings.
  • Provide referrals to any needed services including health, mental health, education, employment, and vocational training.
  • Provide housing stabilization services, which may include topics like money management, life skills, accessing transportation, understanding their lease, self-advocacy and other topics based on their needs.
  • Adopts a “whatever-it-takes” approach in assisting clients to retain housing and independent living.
  • Provides quality assurance and completes quarterly client file reviews to ensure the program meets compliance requirements.
  • Provides interagency and intra-agency consultation, communication, coordination, and referrals.
  • Establish and cultivate relationships with landlords and other housing providers to build a portfolio of housing options for participants.
  • In conjunction with the housing specialist, assist families with the identification of appropriate housing and with lease up including negotiation of rental agreement with landlord. On an ongoing basis, act as an advocate with landlords as needed.
  • Enter data in required databases, compile and submit reports that track client status and support services provided.
  • Provide ongoing case management to families following placement.

Qualifications

QUALIFICATIONS

Knowledge, Skills & Abilities:

  • Must be highly motivated and a self-starter. The ability to communicate with and relate to a diverse group of people including clients, community, and other staff. Must have excellent administrative and organizational skills.
  • Knowledge of case management principles and intervention techniques specific to homeless populations or other under-served groups.
  • Knowledge of the housing market/housing programs is desirable.
  • The position requires an ability to work flexible hours including some early mornings and evenings.
  • Bi-lingual Spanish/English desirable.

 

Experience:

  • Demonstrated knowledge of trauma-informed crisis intervention, violence dynamics, domestic violence, intimate partner violence, human trafficking, stalking, child abuse/neglect, stalking, and elder abuse intervention, experience in Crisis Counseling.
  • Ability to establish effective working relationships with internal and external partnering agencies.
  • One to two years of experience providing case management services/advocacy with homeless and low-income families and/or families experiencing domestic violence.

Education:

  • Bachelor's degree from an accredited college or university in social work, human services, psychology, or a related field. A combination of education and experience may be substituted.

Direct Reports:

  • This position has no direct supervisory responsibilities.

Computer Skills:

  • Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook) and widely supported internet browsers.

Certificates, Licenses and Registrations:

  • State-certified 40-Hour Domestic Violence Training Certificate or willing to obtain shortly after hire.
  • Valid California Class C Driver's License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
  • Valid automobile liability insurance.

Travel Requirements:

  • Must be able to travel to clients' homes for home visits when needed and to SJC office at least twice a week.

Status: Full-time/Non-Exempt

Salary: $20/hour to $25.00/hour
